Firstly we have to apply for new work permit – we can obtain the work permit on your behalf and following documents are required:
- Signed application form (see attached, kindly ask you to sign where “podpis zadatele” is mentioned)
- Signed power of attorney (see attached)
- One passport size photo with light background (3,5 x 4,5 cm)
- Notarized copy of your diploma of the highest relevant education nostrified and translated into Czech (we can arrange the certified translation and we might help with obtaining the nostrification, the nostrification is done by a relevant Czech school and might take 1 – 2 months.)
Please be informed that the notarization is new requirement since January 2012. For the nostrification the legalized diploma and program of study is required. Kindly ask you to confirm where the diploma was issued in order to verify whether further legalization will be required.
The labour office takes about 40-50 days to decide about the application so please send me the documents to the address mentioned in my signature at your earliest convenience.
